In order to start a health insurance application or other product/service offering, you must first establish an account by proving certain personal contact information, such as name, address, email, phone number, gender, date of birth, password, etc.

Transactional and Third Party Information
Purchase information, including account activity, may be gathered from us, our affiliates or non affiliated third parties to provide our products and/or services. Information from public sources, such as consumer reporting agencies and verification services may be collected to uphold regulatory requirements, verify data accuracy and prevent fraud. Other sources include financial institutions, such as a bank or broker used to transfer funds into a Lifespring Health Network account.

Health Insurance, Health Saving Accounts or Additional Products
If you apply for health insurance, a Health Savings Account or another product through our website, we may ask you for personal information and/or health related information concerning you and your family members who will be included on your application. In order initiate the application process, this information may be used by your selected insurance company, agent or HSA trustee or administrator. In addition, your credit card or bank information might be required to process your application, collect application fees or insurance premiums upon approval of your application.

To generate a quote or process your application we may be required to use your personal information. At times, we may need to get in touch with you through email or a phone number to seek additional information, inform you of your application status, new product information and/or to survey your satisfaction of our service. To better understand and meet your health care savings needs we may share or obtain personal information with or from our affiliates to inform you about our products, services and promotions. We may use information about you to provide custom offers tailored to your interests. If you do not want to receive offers from Lifespring Health Network or our partner companies, you may opt out of receiving them.

Applying for a health insurance application through us or an insurance agent or partner agency, we may disclose your personal information to process your application. Applying for a Health Savings Account through an authorized HSA trustee or administrator with whom we have partnered, we may disclose your personal information to that trustee or administrator to complete you application.

Service providers may be informed of limited personal information to perform functions on our behalf, such as to send email and standard U.S. postal mail, review analytics, conduct marketing activities, process applications and credit card payments, and service accounts.

Upon becoming a Lifespring Health member, we may use your contact and transaction data to manage your account.

Sponsor and Community Information
You may decide to sponsor a Lifespring Health Community and invite friends and family to join your Community by providing their names and email addresses. You can increase your healthcare savings through Lifespring Health Network by creating a community of family and friends that delivers rewards to you and your community members.

Lifespring Health Network automatically sends an email inviting your family and friends to join Lifespring Health Network. We only send this email once and do not store this information for future purposes.
